白內(nèi)障超聲乳化吸除聯(lián)合人工晶體植入術的并發(fā)癥分析






【摘 要】 目的：探討白內(nèi)障超聲乳化吸除術并人工晶體植術的并發(fā)癥的發(fā)生情況及預防措施。方法：回顧2007～2009年我院進行的89例（96只眼）超聲乳化白內(nèi)障吸除并人工晶體植入術的并發(fā)癥情況，記錄手術中和手術后并發(fā)癥情況，平均隨訪時間（6±12）個月。結果：在89例患者中，96只眼中裸眼或矯正視力≥0.5有85只眼，<0.5者11只眼，最高裸眼或矯正視力視力1.0，最低0.1；0.3＜裸眼視力或矯正視力＜0.5有6只眼；0.1＜裸眼視力或矯正視力≤0.3有4只眼；裸眼視力或矯正視力0.1的1只眼。術中并發(fā)癥：發(fā)生后囊破裂4只眼，玻璃體脫出3只眼，人工晶體植入囊袋內(nèi)3只眼，植入睫狀溝內(nèi)1只眼。術后1個月裸眼或矯正視力≥0.5者2只眼，＜0.5者2只眼。術后并發(fā)癥：輕、中度角膜水腫21只眼，均在7日內(nèi)消退；重度角膜水腫5只眼和IOL前膜形成3只眼，術后1個月矯正視力＜0.5。結論：超聲乳化白內(nèi)障吸除術聯(lián)合人工晶體植入術可引發(fā)一系列手術并發(fā)癥，只有針對患者白內(nèi)障類型和晶狀體核硬度等情況正確處理，患者術后仍可獲得較好的視力恢復。

Analysis of complicatons of phacoemulsification and IOL implantation   Zhu Ben-hu, Yang Wen-zhon. People's Hospital of Deqing County , Zhejiang Province  313200 ,China

[Abstract]  Object To explore the causes and the preventive measures of the complication of phacoemulsification and intraocular lens implantation. Methods  Retrospective analysis was carried out on the main complication of 89 cases(96 eyes) of phacoemulsification and intraocular lens implantation from 2007 to 2009. The intraoperative and postoperative complication were recorded.The average  follow-up period was (6 ± 12) months. Results  In 96 eyes of 89 cases,uncorrected or corrected  visual acuity:85 eyes≥0.5,11 eyes<0.5,the best visual acuity was 1.0,the worst was 0.1, 0.3＜6 eyes＜0.5, 0.1＜4 eyes≤0.3, 1 eye=0.1. The intraoperative complications included:posterior capsule rupture(4 eyes), vitreous prolapse(3 eyes), intraocular lens implantation in the capsular bag(3 eyes), implantation in the ciliary sulcus(1 eye).The uncorrected or corrected  Visual acuity after one month:2 eyes≥ 0.5,2 eyes<0.5.The  postoperative complications included :mild to moderate corneal edema(21 eyes), was dissipated within 7 days; severe corneal edema (5 eyes ),and IOL membrane formation (3 eyes), visual acuity <0.5 after one month. Conclusion There may be a series of complications happened in phacoemulsification combined with intraocular lens implantation . Only  to aim directly at cataract type and lens nuclear hardness of patients, etc,the proper management is performed, patients can achieve better visual outcome.
